Primary Care Support Service
The Primary Care Support Service (PCSS) works in partnership with GP’s and primary care staff across the London Borough of Hammersmith & Fulham, the Royal Borough of Kensington and Chelsea, and the City of Westminster. We offer a free, friendly and confidential service which is open to people aged 18 or above who have alcohol or drug problems.
PCSS also provides advice and information to families, friends and carers. In participating surgeries your GP, Nurse or Health Care Worker can make an appointment for you with the Shared Care Worker who will provide you with information about what we can offer and will undertake an assessment of your needs. We will work together to formulate a care plan tailored to your needs, supporting you to reach your full potential.

Self referrals: If you are concerned about your alcohol or substance misuse issues then you should speak to your GP or Practice Nurse in the first instance.
Onward referrals: Depending on need, we can refer service users to community or residential detox, day programmes or residential rehab.
